In order to save you time and better serve you, when you schedule your initial consultation, our staff will email you a link that allows you to fill out all health history and HIPPA forms online in a safe, secure format. Then, on your consultation day, you’ll be ready to jump right in with x-rays, an exam and a chat with Dr. Patel and a treatment coordinator.
